The cemetery is located in Mayfield on the north side of E. Cemetery Road with entrances at its intersection with N 7th Street and N 6th Street. The cemetery has entrance monuments, called the "Confederate Memorial Gates in Mayfield", that were gifted to the cemetery by the United Daughters of the Confederacy. The monuments consist of three pairs of stuccoed concrete posts. Gates are connected to the set of posts at the main entrance. When closed the gates read "The United Daughters of the Confederacy Memorial". The cemetery is also the site of the Woolridge Monuments.
In June 1912, amid the implementation of plans to beautify and improve the cemetery, the Cemetery Association solicited ideas for a new name as City Cemetery was no longer considered to be a proper name. In July the Association announced the adoption of the name Maplewood Cemetery. That year roads and paths within its grounds were graded and laid with gravel to solve persistent issues with horses, carriages, and wagons becoming mired in mud, and a perimeter fence was erected to prevent farm animals using the land as pasture.
The U.S. Geological Survey (USGS) Geographic Names Information System (GNIS) feature ID for the cemetery is 497524 (Maplewood Cemetery).
