The Oak Creek cemetery, originally known as Crown Hill Cemetery, was purchased and laid out in the summer of 1910. The original cemetery 2 acre property was purchased and named by William E. Hosking, Undertaker, Oak Creek. The first official burial in this Cemetery was in February of 1911 with the burial of Otto Maurer. The year 1911 appears on the gates of the cemetery.

The name Crown Hill Cemetery was constantly confused with the Crown Hill Cemetery in the metropolitan Denver, Colorado area, by about 1929 the use of the name Crown Hill Cemetery, Oak Creek, had virtually died away.

Officially the name of the cemetery became Oak Creek Cemetery in 1936 when the town of Oak Creek, Colorado purchased the cemetery.

Additional cemetery area was added on Sept. 16, 1929, when an adjoining property owner The R. C. Packing Co. sold 1/2 acre to the Neighbors of the Woodcraft #803. The "Woodmen of the World" cemetery had no known markings for the cemetery area.

In November 1960 the Routt County Board of County commissioners created the Oak Creek Cemetery District. In 1962 the Town of Oak Creek ownership was transferred to the Oak Creek Cemetery District. Later on May 8, 1962, the Neighbors of the Woodcraft #803 deeded their land (1/2 acre) to the Oak Creek Cemetery District.
